Obtain a referral from a physician or nephrologist at your nearest government hospital. Visit the Eskag Sanjeevani unit with your referral, Aadhaar card, and recent blood reports. BPL cardholders should also carry their card for scheme eligibility verification. Registration and slot allocation are completed on the same visit.
